What is Vision Supplemental Insurance?*

The SureBridge Premiere Vision plan provides a routine eye exam for each insured person, per 12 month period, with no co-pay. There is a $10 co-pay for frames, uncoated plastic lenses, or corrective contacts up to a $120 allowance. Through EyeMed Vision Care’s SELECT network, thousands of retail providers also provide additional savings for eyewear needs.
Who Qualifies?

All U.S. citizens and legally present residents who are not imprisoned are eligible to purchase an Accident Supplemental Insurance in the state in which they live.

Most plans DO NOT offer additional benefits which should be purchased individually (e.g. dental, hearing and/or vision.)

Eligibility for enrollment may be affected by medical history or pre-existing condition.
